What are the causes for nipple discharge?

In most cases, the cause is not really anything to worry about and is actually due to the widening of the milk ducts (or duct ectasia) that can be seen with age, or due to wart-like growth in the milk duct (or intraductal papilloma). Even the abscesses of the breast can manifest as the discharge of the nipple.
However, it is scarcely possible to find early cancer cells. This will certainly warrant further treatment and follow-up.

Procedure for Microdochectomy surgery

Microdochectomy is done to control the discharge of the nipple, which requires just one duct. It is a straightforward outpatient procedure that can be done under either local or general anesthesia. Prior to surgery, the affected duct is detected by galactography, a technique that examines the ductal system of the breast and acts as a map of the ducts to locate the affected duct. In addition, a variety of examinations, including mammography and breast ultrasound, may be required by the physician in a preoperative manner.

At the operating theater, gentle pressure is applied on the nipple to locate the opening or opening of the affected duct. The fine probe is cautiously placed into the duct as far as possible, ensuring that it is not damaged or disturbed. Then the duct is dilated and the dye is poured into it, labeling the duct.

The borders of the nipple are traced and incised. The areolar skin is lifted to create a skin flap. The infected duct is gently dissected and released from the surrounding tissues for approximately 5 cm. The duct would then be transected and removed. Some surgeons may prefer to implant a drain, which will be removed after a few hours. The incision is closed using absorbable sutures.

Microdochectomy is both a diagnostic and a therapeutic procedure. The specimen collected shall be sent for biopsy to ascertain the cause of the discharge of the nipple. If only one duct is involved, the microdiscectomy will result in the resolution of the discharge of the nipple. However, if several ducts are involved, a more comprehensive procedure, such as barium resection or central duct excision, may be suggested.

How do we diagnose microdochectomy surgery?

A combination of medical tests, including mammography, breast ultrasound, will help physicians assess the diagnosis and prepare for proper treatment. Centered on the underlying mammary anatomy, microdiscectomy is the recommended treatment for the discharge of the nipple.

What are the benefits of doing a microdochectomy surgery?

The advantage of breast duct excision is that the tissue removed may be investigated to diagnose the cause of the discharge of the nipple.
